Why Canadians are more susceptible to MS

In an article by Maclean's writer Anne Kingston of Canada, the multiple sclerosis (MS) rate in Canada is very high. It is ranked number one in the world with an estimated 291 cases per 100,000 people. However, Kingston acknowledges that "concern has also been raised that infectious diseases, including Lyme disease, are being misdiagnosed as MS." How many of the present day cases of MS in Canada are actually people suffering from Lyme disease? What would the numbers look like for people who have been misdiagnosed with other diseases, like fibromyalgia or chronic fatigue?
The rebranding of multiple sclerosis raises questions: why are rates so high here?
